Gallbladder carcinoma surgical therapy. An overview.

چکیده
Carcinoma of the gallbladder is the most frequent tumor of the extrahepatic biliary tract, and the fourth commonest upper gastrointestinal malignancy. Of the histologic types, adenocarcinoma is the most frequent (80%) (1,2). Gallbladder cancer probably has a multifactorial etiology. More than 80% of patients diagnosed with gallbladder cancer have gallstones. Karyotipic abnormalities, such as mutation of the tumor suppressor gene p53, which is located on the short arm of chromosome 17, are common in gallbladder cancer. Anomalous junction of the cystic duct and the common bile duct, pancreatobiliary maljunction, cystic disorders of the biliary tree or gallbladder polypoid lesions have been observed frequently in patients with gallbladder cancer (3,4). (gallbladder veins drain directly into the liver bed and are tributaries of the middle hepatic vein), c) lymphatic invasion (9). Loco-regional lymph nodes are disposed along the main arteries and include the cystic duct, pericholedochal, hilar (hepatoduodenal ligament, inferior caval vein), peripancreatic (head), periduodenal, periportal, coeliac, superior mesenteric and para-aortic lymphatic chains (10,11). The TNM classification and stage grouping criteria are useful to predict survival and decide therapy (Tables I-IV) (12).

OBJECTIVE To evaluate the prognostic factors of gallbladder carcinoma. METHODS Presentation, operative data, complications, and survival outcome were examined for 132 gallbladder carcinoma patients who underwent gallbladder surgery in our unit during 2002-2007, and follow-up results were obtained from every patient for univariate and multivariate survival analysis. RESULTS The univariate an...
متن کاملDermatomyositis as the first manifestation of gallbladder adenocarcinoma: case report and literature overview
Dermatomyositis (DM) is characterized by pathognomic cutaneous manifestations (heliotrope rash, periorbital edema, Gottron's papules) and proximal muscle weakness. In this paper, I will present the case of a 48-year-old female patient whose dermatomyositis was initially diagnosed as vasculitis. Following the patient's inadequate response to corticosteroid treatment, clinical and radiologic exam...
متن کاملDiagnostic and management issues in gallbladder carcinoma.
Carcinoma of the gallbladder is a rare malignancy, with an incidence rate in the United States of 2.2 to 4.4 per 100,000 persons. Its clinical presentation is nonspecific, and the majority of patients have advanced disease at presentation. The diagnosis is rarely made preoperatively. About 90% of gallbladder cancers are adenocarcinomas. Surgical treatment is curative for lesions localized to th...
متن کاملCoexisting cancers: a mixture of neuroendocrine carcinoma and adenocarcinoma in the gallbladder
BACKGROUND Neuroendocrine carcinoma is rare with a proportion of less than 2% in gallbladder malignancies, cases of gallbladder neuroendocrine cell carcinoma coexisting with adenocarcinoma are exceptionally rare, and the prognosis is dismal. METHODS Herein, we presented an unusual case of poorly differentiated gallbladder neuroendocrine cell carcinoma coexisting with poorly differentiated ade...
متن کاملComplete Resolution of Metastatic Gallbladder Cancer after Standard Gemcitabine-Cisplatin Combination Therapy
Gallbladder carcinoma (GBC) is a rare and deadly disease. The only curative option is a total surgical resection. If the disease is inoperable, palliative combination chemotherapy with gemcitabine-cisplatin remains the standard of care. We present here a case of a 47-year-old gentleman diagnosed with metastatic GBC who saw a complete resolution of his disease with seven cycles of standard gemci...
